The family of a man who died after being crushed to death at work have spoken of their tragic loss.

William Aveyard’s daughter Helen said after an inquest into her father’s death: “My father went to work as he had done every day for over 33 years. He never came home that night. At the age of 49 he was crushed to death at work. To have lost your father in this way is devastating. As a family we are still coming to terms with his death.” She added that the inquest had made it clear that her father’s employers, Wakefield printers Bezier, were responsible for his death due to shortcomings in health and safety procedures.

Helen was speaking after an inquest jury into the death in May 2008 found that Mr Aveyard had received no training into the use of the machine into which he became trapped after trying to clear a blockage. A colleague had told the inquest that he first noticed something was wrong when the machine, which was used for cutting, scoring and pressing card, made a clattering noise. He then turned and saw Mr Aveyard lying across the machine with his feet protruding. Paramedics were called but the man was declared dead at the scene.

The inquest heard that instructions on health and safety were verbal rather than written and that safety features were not effective.
